Personal Data Protection Law (KVKK) Information Notice and Explicit Consent Form

Within the scope of the psychiatric examination, diagnosis and treatment services provided to you, the following personal data may be processed:

• Identity information (name, surname, Turkish ID number, etc.)
• Contact information (telephone number, e-mail address, home address, etc.)
• Health data (psychiatric history, diagnosis, treatment plan, prescription information, test and assessment results, etc.)
• Appointment and examination records
• Financial data (payment and invoice information)

Your personal data may be collected through physical application, verbally, in writing or electronically, including via GSM technologies.

Your personal data may be processed for the purposes listed below, without being limited to them. Your personal data will be processed where processing personal data belonging to the parties of a contract is necessary, provided that it is directly related to the establishment or performance of that contract; where it is mandatory for the data controller to fulfil its legal obligations; where it is expressly provided for by law; and where data processing is mandatory for the legitimate interest and for the establishment, exercise or protection of a right, provided that it does not harm your fundamental rights and freedoms.

• Providing services to clients,
• Recording and tracking client information,
• Communicating with clients and following up,
• Recording and following up client requests,
• Fulfilling administrative obligations under the relevant legislation, such as the Basic Law on Health Services, the Social Insurance and General Health Insurance Law, the Code of Obligations and the Consumer Protection Law.

The procedures and principles applicable to transfers of personal data are regulated in Articles 8 and 9 of the KVK Law; the personal data and special categories of personal data of the data subject may be transferred to authorised public institutions and organisations and to other relevant third parties.

• request deletion or destruction of your personal data under the conditions set out in Article 7,
• request that third parties to whom your personal data has been transferred be notified,
• object to a result against you arising from analysis of the processed data exclusively by automated systems,
• claim compensation for damages arising from unlawful processing of your personal data.
